Case management and planning
An individual plan that ties housing, health care, benefits and personal goals together.
Comprehensive Community Support Services (CCSS) help adults living with serious mental illness build stable, independent lives. Most of the work happens outside the office: in homes, neighborhoods and the places people spend their days.
FY2026 is July 1, 2025 through June 30, 2026.
Recovery is not only about symptoms. It is about paying rent, getting to appointments, keeping benefits, and having people in your corner. CCSS brings all of that into a single plan built around the person's own goals.
Our community support workers coordinate housing, finances, health care and community resources, and many clients also work with a Certified Peer Support Worker who has walked their own road to recovery.

In homes, neighborhoods and community settings, wherever the person is.
Peer workers show what recovery can look like, because they have lived it.
